The Person Centered Systems Working Group (PCSWG) is coordinated by the Grampians Pyrenees Primary Care Partnership.
The PCSWG provides a forum for partner agencies to learn, share and collaborate on shared care and health reform and system changes to improve person centered care outcomes across the region.
Membership is open to all interested parties working within the health and community agencies across the GPPCP catchment.
