Partnership initiatives

Whenever possible GPPCP works with partner agencies on projects that aim to develop innovative, practical and timely resources for service providers.

In 2017 GPPCP worked with key member agencies to develop, coordinate and distribute 2 separate collaborative design initiatives.

Partnership projects provide a great way for agencies to work together, provide shared knowledge and resources enabling better outcomes.

The Grampians, Who, What and Where of Services for people affected by cancer

Community members and health service providers can now access an online directory for up to date information on cancer related services.

Grampians Pyrenees Primary Care Partnership (GPPCP) in conjunction with Grampians Integrated Cancer Services (GICS), began the online directory in October 2016 and was completed in 2017. The directory is now available on the GICS website at www.gics.com.au

The directory draws live data from the National Health Service Directory (NHSD) and features more than 1200 registered groups and organisations from the South Australian boarder in the west through to Bacchus Marsh in the east.

Community members can utilise the directory for information such as location, opening hours, billing and parking options, pharmacy, hospital, allied health, specialist and community services.

For health practitioners, the directory helps to facilitate the coordination of ongoing care for their patients, providing connection of care and referral pathways for location based services.

The directory also features broader service information such as home visiting, online and phone services and telehealth enabled services.

Shared Care & Local Level Agreements Project Report 2017

In collaboration with Grampians Partners in Recovery (GPIR), this project aimed to develop and test the validity of Local Level Agreements to support electronic and secure shared care between health and community care providers.

A solid foundation could then be established to progress collaboration and embed shared care planning, at a systems level across, the Grampians Pyrenees Primary Care Partnership’s (GPPCP) catchment.
