Our Strategic Plan has been developed with input from our members,
stakeholders and staff, drawing on a careful analysis of population
health and wellbeing data.
The plan has also been conceptualised using the Victorian
Department of Health and Human Services’ program logic for Primary
Care Partnerships and builds on the available evidence base and
best/emerging practice.
