Service Coordination 

Service Coordination is what we bring to our business to ensure the best outcome for our clients. It is recognised by the practices, processes, protocols and systems embedded and utilised within an organisation and in the relationships with other organisations. The main intent of Service Coordination is to improve the outcome for those using the service system by placing them at the centre of service delivery, providing better access to services and opportunities for early intervention and health promotion.


The Better Access to Services framework underpins Service Coordination. It provides a provides a guide to achieving better coordination of services as part of the Primary Care Partnership Strategy and gives service managers and practitioners consistent practices and defines the roles and responsibilities regarding consumers they have in common. There is consistency in practice and common language within the healthcare and other sectors. 

The available resources inform and guide health care professionals, practitioners, case managers and other sector workers on the ‘how to’ of Service Coordination.  The resources include:

At the local level, Grampians Pyrenees Primary Care Partnership notes on its Strategic Directions 2008/2009 that its core business is coordination through partnership.  The action plan for  the Integration of Chronic Disease Prevention and Management 2008/2012 indicates that its core business is “Coordination of Chronic Disease prevention and management strategies and services through partnerships”.

HACC Services    (Home and Community Care)
Many of the member agencies receive HACC funding for a variety of programs.  In recent developments in the delivery of these programs, assessments are now done by agencies designated to perform the Living at Home Assessments (LAHA).  Agencies not designated will make a referral to the LAHA designated agency who will perform the assessment then make referrals to appropriate service providers including the originating referrer for the specific HACC service required.  
The designated agencies in the Grampians Region are:
    Pyrenees Shire
    Ararat Rural City
    East Grampians Health Services (District Nursing)
    Northern Grampians Shire

In addition to the LAHA, the Active Service Model (ASM) is being incorporated into the delivery of service. 

The goal of the Active Service Model is to assist people in the HACC target group to live in the community as independently and autonomously as possible. In this context, independence refers to the capacity of people to manage the day to day activities of their daily life. Autonomy refers to making decisions about one's life. Not all HACC clients will be able to live in the community without some form of assistance, but the goal of this initiative is to ensure that clients are able to gain the greatest level of independence they can possibly achieve, and equally, that they can be as actively involved in making decisions about their life as they can be, such as the type of services they receive and the goals they wish to achieve.
