What is the Primary Care Partnership?

Grampians Pyrenees Primary Care Partnership (GPPCP) is a voluntary alliance of over 30 health, welfare, local government, community based services and disability agencies. GPPCP covers the 3 Local Government Areas of Northern Grampians Shire, Ararat Rural City and Pyrenees Shire.  

Grampians Pyrenees PCP has evolved over the last 8 years to reflect what member agencies and other partners in the catchment have seen as the most important activities against agreed priorities. 

All PCPs are expected to develop partnerships, promote the integration of health promotion across agencies, support integrated chronic disease management and the coordination of services.

Primary Care Partnerships are an initiative of the State Government to create a primary care service system. The main goals of the reform are to:

  • Improve the experience and outcomes of people who use primary care services
  • Reduce the preventable use of hospital, medical and residential services through a greater emphasis on health promotion programs and by responding to early signs of disease and/or people's need for support

The emphasis of the PCP Strategy on health promotion, consumer engagement and community-wide service planning and coordination provides great scope for achieving improved population health outcomes.
